VNA Homecare strives to assist patients, caregivers and physicians in achieving better quality of life and reduction of re-hospitalizations. One of the ways we’re achieving this is through our telehome care monitoring program.
The telehome care monitoring program benefits everyone – patients, caregivers, physicians and referral sources. This means better patient results will be achieved, and delivered more cost-effectively.
The telehome care monitoring program encompasses a variety of in-home equipment that records patient data such as weight, blood pressure, heart rate and oxygen levels. Within minutes, the readings are transmitted through phones lines to clinicians for a complete review. The technology enables clinicians to adjust services to a patient’s changing needs, managing them more efficiently, improving their care, and often decreasing emergency room visits.
If you or someone you love could benefit from telehome care monitoring, please call VNA Homecare at 315-477-HOME (TTY/TDD: 711).
VNA Homecare provides a broad spectrum of services primarily to individuals and families in Onondaga County, with a smaller portion of our programming available to those living in Cayuga, Chenango, Cortland, Jefferson, Madison, Oneida, Oswego and Tompkins Counties as well.
