At VNA Homecare, we understand that properly taking your medications can be confusing, especially if you’re faced with new and unusual medications after hospitalization or surgery. We feel that learning how to take your medication is crucial to your safety. That’s why we offer medication management.
VNA Homecare will provide you with a nurse who has been specially trained to educate patients on medications and assist clients in setting up a specific method of managing their medications on a daily basis that works for them.
This may include a nurse pre-pouring medications or assisting a patient or family member in setting them up. The goal is for each client to be as independent as possible taking his or her medications while insuring all medications are taken as the physician has ordered.
If you or someone you love needs help managing daily medications, VNA Homecare can help. Call 315-477-HOME (TTY/TDD: 711).
VNA Homecare provides a broad spectrum of services primarily to individuals and families in Onondaga County, with a smaller portion of our programming available to those living in Cayuga, Chenango, Cortland, Jefferson, Madison, Oneida, Oswego and Tompkins Counties as well.
